NEED A PRESCRIPTION RENEWAL
Contact your pharmacist. He will be able to send us a request directly to your family doctor.
HOW OFTEN I NEED TO SEE MY FAMILY DOCTOR IN ORDER TO KEEP MY FILE ACTIVE
Ideally, you should see your doctor as needed, but if the delay exceeds 5 years, your file will be considered inactive.
MY RAMQ CARD IS EXIRED - CAN I RECEIVE SERVICE WITHOUT FEE ?
To avoid paying for your consultation, please call the RAMQ at 1-800-561-9749 and ask them to send us a certificate by fax at 450-263-6457.

We are not accepting new patients at the clinic. If you are from the Cowansville area and have a minor emergency, you can contact the Primary Care Access Desk (Guichet d'Accès Première ligne - GAP) at 1-877-791-5439,
or call 811, option 3, or use the Primary Care Access Desk online at https://gap.soinsvirtuels.gouv.qc.ca/en to book an appointment with a healthcare professional.
DO I NEED A REFERRAL FOR MY BLOOD TEST OR RADIOLOGY APPOINTMENT?
It is your responsibility to keep your referral in a safe place and bring it to your appointment.
WHAT IS THE PROCEDURE TO BOOK AN APPOINTMENT IF I AM ENROLLED IN YOUR CLINIC'S GROUP REGISTRATION?
All patients enrolled in group registration must go through 811, option 3, or use the Primary Care Access Desk at https://gap.soinsvirtuels.gouv.qc.ca/en
DO I NEED TO SEE THE RECEPTIONIST IF I HAVE CHECKED IN USING BORIS KIOSK?
If you have no changes to make to your file, you do not need to see the receptionists. Simply take a seat in the appropriate waiting room.
CAN I PICK UP A MEDICAL DOCUMENT FOR ANOTHER PERSON?
Please bring a written and signed authorization to pick up medical documents on behalf of another person.
